First Visit

Your initial appointment will consist of a clinical and radiographic evaluation. A detailed discussion of your condition, treatment options, and recommended treatment plan will take place. Radiographs may be taken in case current ones were not available.

Every effort will be made to see you in a timely manner.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and dental history
  • Radiographs, CT scans
  • A list of any medications you are taking

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).

Please let us know if you have a medical condition that may require premedication with antibiotics prior to dental appointments (such as artificial heart valves, artificial joints).

If you have dental insurance, please bring your current insurance card (we will need the subscriber number, date of birth, group number, and phone number).

IMPORTANT: All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.

X-Rays

If your dentist or physician has taken X-rays, please have them forwarded to our office as they may be helpful in our diagnosis. A digital radiograph can be emailed to us. If additional films are necessary, they can be taken at our facility.
